COVID-19 Reporting, Effective January 10, 2022
In an effort to streamline the process of reporting information related to COVID-19, the District is revising how we collect information. Moving forward, please use this form to:
- Report positive COVID-19 test results OR
- Report exposure to COVID-19 OR
- Report COVID-19 symptoms
After Day 5, students must submit a NEGATIVE result here in order to return to school.
